Patient Participation Group

PPG

Patient Participation group (PPG) are a small group of volunteers trying to represent views of the 15,000+ patients here at Frome Valley Medical Centre. The PPG meet once a month with the Strategic Practice Manager, Vicky Elliott and a GP to exchange ideas or solutions to a problems patients or the practice may have.

Can't Attend?

For Those that can't attend the monthly meetings or have limited time, we have an email group who receive a shortened form of the minutes of the monthly meetings. You can also raise topics and contribute to discussions.

Like to Join?

If you would like to be part of the PPG either in person or by email please complete a short form which can be found on the link below and can be returned via email to fvmcppg@gmail.com . Alternatively if you don't have internet access, you can ask a member of reception for a form to complete and they will then pass it on to a member of the PPG who will then be in contact with you.

Current Projects

One of the PPG's current projects, after speaking to young mums in the surgery, is to find out if there is a need for them to set up a 'drop in' self support group. If you are interested in giving over your views, please download the 'New Parent Questionnaire' from the link below and return via email to fvmcppg@gmail.com  . However, you can pick up a questionnaire from Frome Valley Medical centre and return it to a member of staff at reception.

A brief overview...

The purpose of the Patient Participation Group is to ensure that patients and carers are involved in decisions about the range, shape and quality of services provided by our Practice.

The group meets regularly and  currently consists of 24 patients, the Annual report has recently been completed and is available to view by clicking on the link below. If you would like to become a member of this group please contact the surgery.

 Privacy Notice - How we use your data.

The Patient participation group consists of a number of patients who meet regularly and an email group (PRG). The group holds information on patients who have expressed an interest in receiving a newsletter.

What information do we collect from you?

The following information is held in a secure gmail account:

  • PPG - email addresses, telephone numbers, home addresses.
  • PRG (email group) - email addresses.
  • Newsletter group - email addresses or home address. 

Access to the account is held by the chair and Vice-Chair of the PPG.

From time to time we carry out ad-hoc paper based surveys where will collect patient email addresses, telephone numbers and home addresses. This information will be held for a period of up to 3 months after the survey has been completed. After this time, patients' information will either be transferred to one of the above groups or destroyed.

Who might we share your information with?

Your contact details may be shared with other members of the PPG and, on occasions, with members of the practice administration team.

Your individual rights!

You have the following rights with regard to your data:

  • The right of access to your personal data which the group holds about you.
  • The right to be forgotten (erased) - to request your personal data is erased where it is no longer necessary for the group to retain such data.
  • The right to withdraw your consent at any time.
  • The right to rectification (correction) - to request that the PPG corrects any personal data if it is found to be inaccurate or out of date.
