New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form (GMS1) which needs to be signed and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We also ask for evidence of ID to be supplied when possible. as there are practical reasons why we might need to be assured that people who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us. 

Forms of ID that are helpful are : 1) Photographic ID e.g. Passport/Driving licence.   2) Proof of address e.g. utility bill. 

Medical treatment is available once you have provided both forms of ID and the registration has been processed. Please contact reception for further information.
