Our Practice phone lines remain open for anyone with accessibility needs who might find difficulty completing the form.

The submitted forms will be read during Practice hours. The forms will not be read at weekends or Bank Holidays. They will not be read Monday-Friday between the hours of 17:30 - 08:00. Please contact 111 during out of hours or 999 if this is an emergency.

Patient access and NHS app are also great ways to use our services, including online prescriptions and checking your results.
